Appeal Form

 

A coach or a board member must fill out this form immediately after a fine has been assessed or a suspension/expulsion has occurred. Coaches and city board members are the ONLY individuals who have the right to appeal a hearing decision. Parents and other adult personnel affiliated with a City DO NOT have hearing privileges at the Conference level. This form has to be completed entirely and faxed to the President of the Diablo Valley Conference within 7 days after the original ruling to be valid. Any fine assessed must be paid and mailed to Diablo Valley Youth Football Conference, Attention President, P.O. BOX 2949 CASTRO VALLEY, CALIFORNIA 94546. The Executive Board of DVYFC is NOT required to hear the appeal unless the Board feels it is warranted. If the Executive Board agrees to a hearing it will take place within (7) days of the receipt of this form and any new evidence. Please consult the DVYFC Rule Book article 15 for all rules governing the appeal process.

An appeal may be filed ONLY after the levied fine has been paid. The fine amount could be returned if the original decision is reversed or reduced. This form and any fine must be received within (7) days of the date of the original decision. NO APPEALS WILL BE HEARD IF THIS TIME FRAME IS NOT MET.
