Participant Drop Form

In the event that a football player or cheerleader is dropped from a team after the certification process, this form is to be completed, and the I.D. card mailed to the Diablo Valley Youth Football Conference, P.O. Box 2949 Castro Valley, California 94546 within 5 days of the participant being dropped. A copy of the drop form must be placed in the Cheer squad book or the Football weigh book. If for any reason this is not completed a fine or expulsion of Head Coach may be accessed to the city.
